Job Description

Oak Street Health is seeking a Primary Care Physician (Spanish Preferred) to provide comprehensive, high-quality primary care to adult Medicare patients in underserved communities. In this role, you will build long-term relationships with a dedicated patient panel, focusing on prevention, chronic disease management, and closing care gaps to improve health outcomes. As a Primary Care Physician, you will deliver evidence-based, team-oriented care in a value-based model, partnering closely with nurses, medical assistants, care managers, social workers, and behavioral health specialists. You will conduct thorough adult primary care visits, perform routine screenings, manage complex chronic conditions, coordinate specialist referrals, and provide timely follow-up to reduce avoidable hospitalizations and emergency department use. You will use population health tools and electronic health records to track quality metrics, identify high-risk patients, and proactively reach out to support their care plans. You will communicate clearly and compassionately with patients and caregivers, ensuring they understand diagnoses, treatments, and preventive care recommendations. Spanish language skills are strongly preferred to better serve a large Spanish-speaking patient population and to enhance access and cultural responsiveness.
