Oak Street Health is seeking a Primary Care Physician to join our Chicagoland clinics within our Healthcare & Medical Services department. As a member of a mission-driven team focused on adults on Medicare, you will provide comprehensive, patient-centered primary care to underserved communities across the Chicagoland area. You will deliver high-quality outpatient care through longer visits, smaller patient panels, and a strong emphasis on prevention, chronic disease management, and care coordination. In this role, you will manage a panel of adult patients with a wide range of chronic and acute conditions, perform thorough assessments, develop personalized treatment plans, and coordinate with an interdisciplinary care team that may include nurse practitioners, nurses, social workers, care managers, and other specialists. You will leverage evidence-based guidelines, population health tools, and data-driven insights to improve patient outcomes, reduce hospitalizations, and enhance quality of life for your patients. You will work in a collaborative, inclusive environment where your voice is valued and innovation is encouraged. Oak Street Health emphasizes a team-based model of care in which physicians actively participate in care huddles, case reviews, and quality improvement initiatives. You will help shape clinic workflows, contribute to a culture of continuous learning, and mentor or support other clinical team members as needed. Daily responsibilities include performing in-person primary care visits in the clinic, conducting follow-up appointments and chronic disease check-ins, ordering and interpreting diagnostic tests, prescribing medications, documenting thoroughly and accurately in the electronic health record, and ensuring appropriate preventive screenings and vaccinations. You will also communicate regularly with patients and caregivers, addressing questions and concerns, and coordinating referrals to specialists or community resources as needed. The ideal candidate is passionate about health equity, comfortable working with older adults and complex medical and social needs, and committed to relationship-based care.
