Oak Street Health is seeking a compassionate, bilingual Primary Care Physician to join our Healthcare & Medical Services team. As a physician within our network of primary care centers for adults on Medicare, you will deliver comprehensive, patient-centered care to underserved communities, with a strong emphasis on prevention, chronic disease management, and population health. In this role, you will manage a dedicated patient panel, perform routine and acute care visits, conduct thorough histories and physical exams, order and interpret diagnostic tests, and develop evidence-based treatment and care plans. You will focus on closing care gaps, improving quality metrics, and supporting patients in managing conditions such as diabetes, hypertension, heart disease, COPD, and other common chronic illnesses. You will use your bilingual skills to communicate clearly with patients and families in their preferred language, ensuring understanding, trust, and adherence to care plans. You will work in a highly collaborative, team-based environment alongside nurses, medical assistants, care managers, social workers, and other clinicians to coordinate care and address medical, behavioral, and social needs. You will use data and technology to track outcomes, identify high-risk patients, and proactively reach out to those who may benefit from additional support or preventive services. Documentation in the electronic health record will be timely, accurate, and compliant with regulatory and payer requirements.
