Oak Street Health is seeking a dedicated Primary Care Physician to join our Healthcare & Medical Services team, providing comprehensive, high-quality primary care to adults on Medicare in underserved communities. As a core member of our patient-centered, team-based care model, you will focus on prevention, chronic disease management, and building long-term, trusting relationships with your patients. You will manage a smaller patient panel than traditional practices, allowing you the time to understand each patient’s medical history, social needs, and personal goals. In this role, you will conduct routine and acute primary care visits, perform thorough histories and physicals, order and interpret diagnostic tests, and develop evidence-based care plans tailored to the needs of an older adult population with multiple chronic conditions. You will coordinate care with nurses, medical assistants, social workers, behavioral health specialists, and care managers to address the full spectrum of medical, behavioral, and social determinants of health. You will use population health tools and electronic medical records to proactively identify care gaps, close quality measures, and reduce avoidable hospitalizations. You will practice in a supportive, inclusive, and collaborative environment where innovation is encouraged and every team member is empowered to make a meaningful impact on community health. You will participate in case conferences and huddles, contribute to quality improvement initiatives, and help shape practice workflows that enhance patient access, safety, and satisfaction. As a Primary Care Physician with Oak Street Health, you will be expected to provide culturally competent care, communicate clearly with patients and families, and advocate for resources that support patients’ social and functional needs. Experience with older adults, complex chronic disease, and value-based care models is strongly preferred.
