Job Description

Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Healthcare & Medical Services team, dedicated to serving adults on Medicare in underserved communities. As a Primary Care Physician with Oak Street Health, you will provide comprehensive, patient-centered care focused on prevention, chronic disease management, and improving overall health outcomes. You will build long-term relationships with a panel of adult and senior patients, conduct thorough evaluations, diagnose and manage acute and chronic conditions, develop personalized care plans, and coordinate with a multidisciplinary care team including nurses, medical assistants, care managers, behavioral health professionals, and community health workers. In this role, you will deliver high-quality outpatient primary care through in-person visits and, when appropriate, telehealth encounters. You will perform routine wellness visits, medication management, and preventive screenings, as well as manage complex comorbidities common in older adult populations such as diabetes, hypertension, COPD, heart disease, and other chronic illnesses. You will utilize evidence-based guidelines, population health tools, and data analytics to identify care gaps, reduce avoidable hospitalizations, and promote proactive, preventative care. You will engage families and caregivers, communicate clearly and empathetically with diverse patient populations, and advocate for resources that address social determinants of health. You will be responsible for accurate and timely documentation in the electronic health record, appropriate use of diagnostics and referrals, and adherence to all regulatory and compliance requirements, including Medicare and value-based care standards. You will help drive performance on quality metrics, patient satisfaction, and clinical outcomes, while maintaining a manageable schedule that supports thorough, unhurried patient visits.
